Overview
The Great State of North Carolina has now made it official: the Shag is
North Carolina’s “Most Popular Dance” and to show everybody how proud you
are to be a shagger, you can display your new North Carolina D.M.V. “SHAG
TAG”. Your license plate logo tells everyone that “I’d rather be shaggin”.
Be one of the first 300 in the state to own this prestigious plate. To
order your tag send in $15.00 for a numbered tag or if you want it
personalized it will be $45.00 (don’t forget to send your application). You
get 4 spaces to be creative and say what you want to say.
Remember that $5.00 of your license plate fee will be donated to the
Hall of Fame Foundation
to help shaggers in need. Hurry up and get your applications sent in before
the first 300 are gone! 177 have already been sold and the count down is
running on that first 300!!!

| Fill in all the blanks on the application. The DMV needs your
telephone numbers in case there is a question about your application. It
will take them less time to call you than to send your application back
through the mail.

| The date line in the bottom right corner of the application needs the
date that you mail the form.

| Be sure and sign the application.
|
| Make your check payable to NC Shag Tags and in the memo line of
the check write DMV shag tags.

| Be sure to sign your check. Please do not staple the check or money
order to the application. Do not send cash in the mail!
